This CAFAS Reliability Test is to be used in conjuction with the online CAFAS training developed for current FAS Outcomes customers. (FAS Outcomes is an web-based application that contains the online CAFAS and PECFAS.)

CAFAS Test Instructions 
The CAFAS Rater Reliability Test is a fillable form, which will allow you to type your answers into it. Once you've completed the test, save the document and see the instructions on the first page for emailing or faxing your completed test to Functional Assessment Systems.  

Taking this quiz and passing it will allow you to attain the status of "CAFAS Certified Rater^®," for which we will send you documentation.
